Why do Urdu novel titles have so many English spellings?
Because Roman Urdu has no single official mapping. Ezafe (the “e” in Peer-e-Kamil), ain, and long alif get typed differently in search. Google still needs one document to extract. Lafzora puts every attested spelling of one work on that work’s URL.

When do similar Roman titles stay on different URLs?
When the writer or the Urdu title differs. Yaram (یارم, Sumaira Hameed) is not Ishq-e-Yaram (عشقِ یارم, Areej Shah) and not Rooh-e-Yaram. Ramz-e-Junoon, Lams-e-Junoon, and Junoon-e-Raqsam share “junoon” and keep three URLs because three writers wrote three books.
Do writer names get the same rule?
